Website privacy
When you submit a trial request, the form sends the information you provide to Phantom Jiu Jitsu Academy's existing GymDesk account and sends a notification to the staff inbox. The team uses it to contact you and schedule your trial class.
Information collected
For an adult trial, the form asks for the participant's name and date of birth. For a kids trial, it asks for the child's name and date of birth plus the parent or guardian's name. Both paths collect an email address, phone number, ZIP code, training interests, experience, preferred training frequency, and any notes you choose to share.
How it is used
Phantom uses the information to answer your questions, arrange your visit, and administer your trial request. GymDesk and the email delivery provider process the submission on Phantom's behalf.
Current tracking
This website uses Google Analytics to understand website traffic and records a lead event after a trial request is accepted. That analytics event does not include your name, email address, phone number, date of birth, or ZIP code.
